Name and local number of union hall (if applicable)
If you served in the military during the last 18 months, provide your member-4 copy of your DD214.
If you worked for a federal employer during the last 18 months, provide your SF-8 and SF-50.
You will also need to fulfill all eligibility requirements.
To be considered “eligible,” you must:
- Be able and available to go to work.
- Be actively seeking work.
*Some individuals may be exempted from this requirement.
- Report any pay you receive while unemployed.
- Be unemployed through no fault of your own.
You may file an initial UI claim online or over the phone.

To file a UI claim over the phone, call 1-866-783-5567.
You will need to certify benefits weekly to keep receiving payments. You can process your weekly claims online or over the phone through the same methods provided above.
If you are going through an appeal process, you still need to certify for UI benefits each week. Instructions will be provided to you.
To file weekly claims, you will need:
- Personal Identification Number (PIN)
- Social Security number.
Reporting or omitting information to qualify for UI benefits is punished by the law. Attempts to claim and receive benefits fraudulently can result in loss of benefits, fines or imprisonment.
