What Documents Do I Need to File an Unemployment Claim in Kentucky?

To file an Unemployment Insurance (UI) claim, you will need to provide proper documentation and information.

You need:

  • Social Security number
  • Mailing address

You also need information regarding your job(s):

  • Name, address and phone number for each of your employers over the last 18 months
  • The exact day you began work at each job
  • The last day you worked at each job

All individuals need to fulfil eligibility requirements as well.

To be considered “eligible,” a claimant must:

  • Have sufficient earned wages.
  • Be unemployed through no fault of your own.
  • Be able to work.
  • Register for work with the Office of Employment and Training (OET).
  • Accept referrals from OET to suitable employment.
  • Participate in other reemployment services and case management.
  • Seek employment on your own.
  • Accept suitable employment when offered.

You may apply for UI benefits online or over the phone. Once you do, you will need to file for benefits every other week to keep receiving benefits.

When filing your bi-weekly claims, you will need the following information and documents:

  • Social Security number
  • Personal Identification Number (PIN)
  • Date you returned to full-time work, if you have returned to work
  • Number of hours you worked, if you were paid or will be paid
  • Gross amount as well as any holiday, vacation, or other pay you received or will receive


Reporting or omitting information to qualify for UI benefits is punished by the law. Attempts to claim and receive benefits fraudulently can result in loss of benefits, fines or imprisonment.
