To apply for Unemployment Insurance (UI) benefits, you must provide all pertaining documents and information.
You will need:
Social Security number
Alien registration number (if applicable)
Names, addresses and telephone numbers of any employers for whom you worked during the last 18 months
Name and local number of union hall (if applicable)

If you served in the military during the last 18 months, provide your member-4 copy of your DD214.
If you worked for a federal employer during the last 18 months, provide your SF-8 and SF-50.
You will also need to fulfill all eligibility requirements.

You will need to certify benefits weekly to keep receiving payments. You can process your weekly claims online or over the phone through the same methods provided above.
If you are going through an appeal process, you still need to certify for UI benefits each week. Instructions will be provided to you.
